There is the same problem here: there were plenty of warnings, but people were not trained in some way to follow the warnings, or they did not have the backbone to really follow their instincts and say, 'There is something fishy going on here, and it is my job, as the inspector, to get to the bottom of it, otherwise a child could die', which is what happened in the end.
