The Subcommittee also recommends that the State party take the necessary steps to ensure the existence of both institutional records (maintained by each prison) and central records (maintained by the state or federal government) of deaths, including the name and age of the deceased, the place and cause of death, the findings of the post-mortem examination and information on the circumstances surrounding the death in the case of deaths not found to be the result of natural causes.

El SPT también recomienda al Estado parte que lleve a cabo las medidas necesarias para que se establezcan registros médicos locales (en la propia institución) y centrales (registro estatal/federal) para las personas fallecidas, con nombre, edad, lugar de fallecimiento y causa de la muerte, información del examen post mortem y las circunstancias en el caso de una muerte no natural.

Подкомитет рекомендует также государству-участнику принять необходимые меры для создания местной (собственно в учреждениях) и центральной (на уровне штата/федерации) систем медицинского учета скончавшихся заключенных с указанием фамилии, возраста, места и причины смерти, с информацией о данных вскрытия и описанием обстоятельств смерти, если она не носит естественного характера.
