The application shall be drawn up on the set of forms provided by the Central Patent Office and shall contain the following particulars: (a) the surname, given name, nationality and domicile or elected domicile in Italy of the applicant, and of his agent if any; (b) the designation of the topography in the form of a title stating, in general terms, the function and the range of applications of the product in which the topography is intended to be incorporated; (c) the tax code of the applicant and/or the agent if he/they reside(s) in Italy; (d) where applicable, a request that publication under Section 10(3) of Law No. 70 of February 21, 1989, be deferred; (e) the signature of the applicant or of his agent; (f) where applicable, the date of the first act of commercial exploitation.

La demande doit être rédigée sur la formule à plusieurs feuillets établie par l’Office central des brevets et contenir les indications suivantes : a) le nom, le prénom, la nationalité et le domicile ou le domicile élu en Italie du déposant et de son mandataire le cas échéant; b) la désignation de la topographie, sous forme de titre indiquant, en substance, la fonction et la portée d’application du produit dans lequel la topographie est destinée à être incorporée; c) le code fiscal du déposant et/ou du mandataire, s’ils résident en Italie; d) le cas échéant, la demande tendant à ce que la publication prévue à l’article 10.3 de la loi No 70 du 21 février 1989 soit différée; e) la signature du déposant ou de son mandataire; f) le cas échéant, la date du premier acte d’exploitation commerciale.
