[15] On May 5, 2003, Mr. Holt's physician gave him clearance to travel to Cuba, as planned, subject to the direction that he elevate his left leg for the duration of the flight to prevent deep vein thrombosis (DVT) and wear an "air cast" on his left leg during the flights.
