Falls ebenfalls eine Duodenal-Obstruktion auftritt führen wir eine laparoskopische GE durch, mit folgender Technik: Installation von 5 Trokars (1 × 18, 3 × 10, 1 × 5 mm); Identifikation einer Jejunum-Schlinge (JS) 30 cm distal Treitz; antecolisches Hochziehen derselben; kleine Eröffnungs-incision am Magenantrum großkurvaturseits und antemesenterial an der hochgezogenen JS; Einbringen des linearen Endonahtklammergerätes (60 mm) bzw. dessen Branchen in Magen und JS; Auslösen der stapler-Anastomose; Verschluß der stapler-Öffnung durch einreihige, fortlaufende, extramuköse Naht mit monofilem resorbierbarem Faden; Ansetzen von (PDS-) Clips an den Fadenenden anstelle von Knoten; Anlegen einer Seit-zu-Seit Anastomose am Fußpunkt der Ω-JS mit derselben Technik (30 mm linear stapler).

If duodenal tumor obstruction is present, laparoscopic GE is performed as follows: placement of 5 trocars (1 × 18; 3 × 10; 1 × 5 mm); Identification of a jejunal loop (JL) 30 cm distal Treitz; antecolic placement of the JL; small incision on the large curvature side of the antrum and on the anti-mesenteric side of the JL; insertion of a 60 mm endoscopic linear cutter with its legs in the antrum and JL; creation of the stapled anastomosis; closure of the stapler incision with a single row, continuous, extramucosal absorbable monofilament suture; placement of resorbable (PDS-) clips at the suture ends instead of tying knots; at the foot-point of the Ω-JL a side-to-side anastomosis is performed with the same technique (30 mm linear stapler).
