The diagnosis of pneumorrhachis is difficult or even impossible to make clinically. The occurrence of air in the epidural or subarachnoid space should be distinguished, as they are associated with different pathophysiologic mechanism and causes, which may require emergent treatment. Subarachnoid air is a marker of severe underlying injury.

Het is vaak moeilijk maar wel belangrijk bij een traumatische pneumorrhachis te differentiëren tussen epidurale en subarachnoïdale lucht, omdat elke situatie geassocieerd kan zijn met een verschillende oorzaak, waarbij subarachnoïdale pneumorrhachis vaak een marker is van ernstig (schedel/hersen)letsel.
